Employment Type: Full time Shift: Day Shift Description: In association with the clinical leadership team, and through a collaborative role with other members of the interdisciplinary healthcare team, and according to established policies and procedures, the CNL role provides horizontal leadership accountable for the management of care and the care environment through knowing the patient, communication and lateral integration of care. Serves as a change agent to translate and integrate evidence into practice; to design and evaluate care and improve outcomes. In addition to generalist preparation in nursing with enhanced clinical skills, the CNL is primarily unit or setting based and is responsible for coordination of care for a specified cohort of patients within a micro-system. The number of patients for which the CNL is responsible depends on the health care setting and patient nature and complexity. The mix of nursing personnel required in acute care is a function of the nature and quantity of work that must be accomplished on a particular unit. In his or her day-to-day practice, the CNL develops an overall view of patients, families and caregivers and establishes strong partnerships with them to deliver optimal care. The CNL has 24-hour accountability for the care provided by self and others to whom care is delegated. Why Saint Joseph Health System?

    • Actively demonstrates the organization's mission and core values, and conducts oneself at all times in a manner consistent with these values.

    • Clinician: Designer/coordinator/integrator/evaluator of care to individuals, families, groups, communities, and populations: able to understand the rationale for care and competently deliver this care to an increasingly complex and diverse population in multiple environments. Performs or oversees the collection of holistic assessment. Carries a caseload of patients dependent upon the setting and nature of the patients; Provides care at the point of care to individuals across the lifespan with particular emphasis on health promotion and risk reduction services, utilizing the best available evidence.
    • Outcomes Manager: Synthesizes data, information and knowledge to evaluate and achieve optimal client outcomes; Collects data on patient outcomes on the unit i.e. pressure ulcers, medication errors, falls etc.; Maintains population / unit level metrics and dashboards; Uses metrics with work team to identify areas of opportunity and facilitates process and patient outcome improvements; Member/chair of unit or setting based quality or performance improvement group
    • Advocacy: Adept at ensuring that clients, families and communities are well-informed and included in care planning and is an informed leader for improving care; Serves as an advocate for the professional and the interdisciplinary health care team;
    • Educator: Uses appropriate teaching principles and strategies as well as current information, materials and technologies to teach clients, groups and other health care professionals; Creates or coordinates the development of patient educational pathways and patient education materials; Provides or oversees the education of the patient and family to understand illness and self care concepts; Consciously provides opportunities for less experienced nursing staff to observe and participate in physical assessment, data review and evaluation of individual outcomes provided by the CNL; Engages the staff in clinical rounds; Provides formal and informal educational opportunities on the unit to improve clinical skills of all staff.
    • Information Manager: Uses Genesis and other information systems and technology that put knowledge at the point of care to improve health care outcomes and to obtain evidence-based practices and the latest research to provide current best practice information to care providers on the unit; Develops and utilizes tools available to effectively evaluate, summarize and communicate patient's progress toward outcomes to members of the healthcare team and patient and family; Ensures effective communication and flow of information to all appropriate care providers by obtaining appropriate comprehensive patient story and transmitting that effectively to care providers.
    • Micro-Systems Analyst/Risk Anticipator: Able to participate in systems review using LEAN and other tools to improve quality of client care delivers and at the individual level to critically evaluate and anticipate risks to client safety with the aim of prevent medical error; Incorporates standards from regulatory agencies related to patient safety including JCAHO, OSHA, CMS, and others into daily care and team processes; and is involved in safety initiatives at the department and interdepartmental levels to improve care outcomes within the micro-system of care.
    • Team Manager: Delegates and manages the clinical nursing team resources and serves as a leader and partner in the interdisciplinary health care team; and effectively identifies the need for consultation services of Advanced Practices Nurses and other allied health providers; Recommends specific programming to build clinical skill set of staff; Provides input and feedback to Manager related to evaluation and competency of staff; Coordinates the delivery of care of the multiple disciplines actively involved in the patient's care.
    • Member of a profession: Accountable for the ongoing acquisition of knowledge and skill to effect change in health care practice and outcomes and in the profession; Creates opportunities to advocate for the role of the professional nurse as a key member of the interdisciplinary team; Participates in continuing education for self and the development of colleagues and future generations; Serves in formal and informal coach and mentoring roles with staff RNs.
    • Lifelong Learner: Actively pursues new knowledge and skills as one's role and needs of the health care system evolves; Participates in continuing education for self and the development of colleagues and future generations.
    • Models personal and professional growth by continuing education and obtaining certification in an area of specialization.
    • Actively participates in shared governance model for nursing and activities in support of Magnet designation.
    • Manages subordinates in respective department(s). Is responsible for the overall direction, coordination, and evaluation of these department(s). Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
    • Duties & responsibilities include accuracy of documenting services and supplies provided to patients, including those that may produce patient charges. If designated as a "Revenue Lead", additional responsibilities will include revenue reconciliation and charge-error correction as specified by departmental process and hospital policy.
    • Performs other duties consistent with purpose of job as directed. JOB SPECIFICATIONS AND CORE COMPETENCIES Education: Graduate of an approved, accredited school of nursing and completion of a Master's degree in Nursing with a preferred emphasis on the Clinical Nurse Leader role and required competencies. Extensive clinical knowledge of the patient population is preferred but may be acquired over time under mentorship of senior staff. Licensure: Indiana RN License (RN-IN); must complete CNL Certification within 6 months. Experience: Three years acute care nursing experience. Progressive experience in health care including involvement in direct patient care, educating staff, and problem solving of unit or organizational issues. Other Job Requirements: Advanced critical thinking skills. Ability to identify improvement in clinical operations and implement change and willingness to challenge established ways of doing things in a constructive way. Ability to lead, mentor and develop staff. Exemplifies a strong, positive attitude toward internal and external customers and continuous service improvement. Analytical ability to define unit or service procedures, to independently oversee the application of methods, guides and processes, to apply sound judgment in choosing proper course of action among multiple option, to interpret policies, to assists in planning short to mid-range goals and determining measures, and to assess and monitor progress toward unit goal attainment. Conflict resolution skills. Diplomacy, tact and listening skills are required. Ability to prioritize in order to optimize time effectively and efficiently, and problem solve various situations. Must exhibit advanced interpersonal communication skills, both verbal and non-verbal. Must have demonstrated abilities to motivate others. Demonstrates ability to present information that is easily understood by learner, and able to select and modify materials provided to patient and other health care staff based on the learners ability. Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to demonstrate competency in use of spreadsheets, databases, and graphical portrayal of data and information. Ability to use other software required to perform essential functions.
