Job Description: An excellent full-time employment opportunity
is available for a Board Certified Family Medicine or Internal
Medicine physician with Fellowship training or Certificate of Added
Qualifications in Geriatric Medicine to serve as Medical Director
of a Program of All-Inclusive Care for the Elderly (PACE) in
Mishawaka, Indiana. Responsibilities: Provide general medical
direction and supervision of participant care under the Executive
Director’s oversight. Oversee delivery of participant care,
clinical outcomes, and manage the Quality Assurance Performance
Improvement (QAPI) program. Develop and maintain medical policies,
procedures, standards, and protocols. Provide leadership and
supervision to Primary Care Providers (PCPs) and Advanced Practice
Providers (APPs). Lead and guide program colleagues and community
providers. Conduct history and physicals on new participants,
event-based, and periodic reassessments; develop medical treatment
plans and deliver necessary medical services in participants’
locations. Interview and evaluate applicants and their caregivers;
participate in intake and assessment decisions. Discuss medical
conditions and prognosis with participants and caregivers to assist
in care planning. Collaborate across divisions in a shared,
collaborative culture. Participate actively in the
interdisciplinary team, educating and advising on diagnoses,
cognitive and physical function, and treatment options. Engage in
team and family conferences and decision-making processes. Educate
participants and families on health maintenance and chronic
conditions in cooperation with nursing staff. Supervise health care
and treatment programs developed by the interdisciplinary team.
Perform physical exams, order and evaluate labs and diagnostic
tests, and maintain systematic records. Exhibit strong skills in
physical assessment and chronic disease management for frail older
adults. Manage changing priorities according to participant needs.
Participate in provider on-call rotation as required.
