single day. Job Title: Primary Care Physician Location: Multiple
Locations in the Chicagoland Area Role Overview: The role of a
Primary Care Physician within our organization is to deliver
equitable and effective value-based healthcare to local Medicare
patients at our innovative network of neighborhood primary care
facilities. Our physicians work within a single medical center,
supported by extensive care teams, allowing them to concentrate on
enhancing the quality of care rather than merely increasing the
volume of services provided. Our model of value-based care, along

minimize hospital admissions, promote preventive care, and ensure
an exceptional patient experience. We are seeking physicians who
are genuinely motivated to provide outstanding care to older
adults. New graduates are encouraged to apply. Highly qualified
candidates may also be considered for a Center Medical Director
role. Key Responsibilities: Evaluate and diagnose patients at our
local clinics. Oversee and manage primary care services. Prescribe
and administer medications and treatments. Maintain patient records
using electronic medical record systems. Collaborate with regional
and central leadership to achieve health quality objectives. Work
alongside Practice Managers to lead and manage the center's care
team. Perform additional duties as assigned. Required
Qualifications: Medical Doctor (M.D.) or Doctor of Osteopathy
(D.O.) degree. Board Certification in Internal Medicine or Family
Medicine (or board eligible). Active, unrestricted state medical
license. Current DEA license. Authorization to work in the U.S.
Preferred Qualifications: Fellowship training in Geriatrics.
Experience in treating geriatric populations. Familiarity with a
Value-Based Healthcare Model. Experience in collaborative
environments to ensure positive health outcomes. Experience in
outpatient primary care settings. Bilingual skills where
applicable. Work Schedule: Anticipated Weekly Hours: 40 Employment
Type: Full-time Compensation: The expected salary range for this
position is: $174,070.00 - $374,920.00 This range reflects the base
hourly rate or annual salary for all roles within this job grade.
