Job Description Job Description Overview We are seeking an
experienced and compassionate MDS Coordinator to join our clinical
leadership team at our highly rated skilled nursing and
rehabilitation facility in the greater Chicago area. This role is
ideal for a nurse who is highly organized, detail-oriented,
knowledgeable in MDS/PDPM, and committed to ensuring accurate
resident assessments and strong clinical outcomes. What We Offer

within our network Key Responsibilities Complete and manage the
full MDS/RAI process in accordance with state, federal, and CMS
regulations. Coordinate interdisciplinary assessment schedules and
ensure timely submission. Oversee care plans and ensure accuracy of
clinical documentation. Collaborate closely with nursing, therapy,
and administrative teams to support resident outcomes and
reimbursement accuracy. Participate in quality assurance, survey
preparation, and performance improvement initiatives. Provide
education and support to staff regarding MDS and clinical
documentation requirements. Qualifications Active RN or LVN license
in Illinois (required). Prior MDS Coordinator experience in a
skilled nursing facility. Strong knowledge of PDPM, RAI guidelines,
and LTC documentation standards. Excellent communication,
assessment, and organizational skills. Ability to work
independently while collaborating with the interdisciplinary team.
