Management License: State RN License or Compact License Required
Certifications: Preferred Certification in Case Management
Must-Have: Ability to work independently. Ability to identify work
that needs to be done and follow through to completion without
prompting. Ability to work collaboratively with the care team.
Ability to complete a comprehensive patient assessment including
patient status to drive discharge. Ability to drive stay
progression and complete discharge planning in a manner that
expedites patient transitions to the next level of care. Ability to
document in a comprehensive, accurate, and timely manner.
Description: Complete collaborative work with the care team.
Conduct comprehensive patient assessments to drive discharge
decisions. Oversee the progression of patient stays. Facilitate
discharge planning to streamline transitions to the next level of
care. Utilize computerized documentation systems effectively.
Participate in an 8-hour shift schedule, which may include weekend
rotations. Potentially float to other units but remain focused on
inpatient case management tasks. Units may range from 20-36 beds
with one or two RN Case Managers assigned to each unit. Dress in
business casual with a white lab coat or specified colored scrubs.
Engage in department-specific training and orientation to ensure
thorough understanding of duties. Demonstrate comprehensive and
