Hourly Rate: $17.00 – $18.00/hour (based on experience) About Aunt

wellness. Summary Under the supervision of the Care Coordination
Outreach Supervisor, the Care Coordination Outreach Worker supports
the delivery of coordinated healthcare services for patients served
by Aunt Martha’s Health & Wellness. This role completes health risk
assessments, conducts outreach, schedules appointments following
emergency department visits or inpatient discharges, and ensures a
high level of patient engagement, service quality, and data
accuracy in a fast-paced, remote environment. Essential Duties and
Responsibilities Maintain patient confidentiality and ethical
standards in accordance with agency policies. Deliver high-quality
customer service using the AIDET model to meet and exceed patient
needs. Utilize the Athena electronic health record system to
document patient data and manage outcomes tracking. Complete health
risk screenings for assigned care coordination patients. Conduct
outreach via phone and virtual engagement; coordinate follow-up
after ED visits or inpatient discharge. Provide health education
focused on preventive care and appropriate emergency department
utilization. Answer designated phone lines, route calls
appropriately, and document messages accurately. Collaborate with
health plans, hospitals, specialists, and physicians to support
comprehensive patient care. Attend all required trainings,
meetings, and Core Curriculum coursework. Perform other duties as
assigned to support program goals. Key Competencies Dependable and

data-driven, solution-focused approach. Ability to work
independently and as part of a collaborative team. Strong written
and verbal communication skills. Excellent organizational skills
and attention to deadlines and compliance. Qualifications Education
and Experience: High School Diploma or equivalent required.
Experience with Microsoft Office programs and data entry required.
Additional Requirements: Valid driver’s license and current auto
insurance. Ability to work evenings, weekends, and scheduled
holidays as needed. Compensation & Benefits $17.00 – $18.00 hourly
