About the Job:

Evaluates, plans provides occupational therapy to patients. Documents patient evaluation, plan of care, progress and discharge status.

What You Will Do:

  • Actively demonstrates the organization's mission and core values, and conducts oneself at all times in a manner consistent with these values. Knows and adheres to all laws and regulations pertaining to patient health, safety, medical information. Complies with regulatory agency (Medicare, JCAHO, CARF etc.) standards and regulations. Conduct evaluations consistent with and appropriate for patient diagnosis and designs treatment plans that are goal oriented, based upon interpretation of evaluation results and patient's personal goals and establishes goals that are functional, measurable and attainable. Performs re-evaluations modifying/updating goals as dictated by patient's progress Assists patient towards goal achievement through effective treatment plan and interventions, administers individual treatment procedures, develops home exercises and self treatment plans for the patient and provides education to patient and family, including diagnosis and prognosis of therapy. Recognizes adverse responses or contradictions to therapy and adjusts treatment accordingly.

Ensures that timely and appropriate discharge plans and recommendations are in place for the anticipated needs of the patient.

Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation (e.g. consent, staffing reports, rounds, attendance agreements, patient and family education needs assessment form) in a specified timeframe.

Documents the occurrence of education by recording occurrence(s) of education and/or home exercise programs provided and reports patient/family return demonstration and/or comprehension of training provided.

Adheres to documentation requirements by regulatory agencies such as Medicare. (Eg. Records duration of specific treatment procedures in minutes).

Submits daily charges that accurately reflect documentation within a specified time frame and assures that documentation is complete prior to discharging patient.

Responsible for participating in staff orientation, patient staffing and team meetings. Makes appropriate recommendations and referrals for additional services. Consults with other professionals and/or appropriately seeks assistance from co-workers as needed. Maintains flexible hours of availability to meet department needs. Engages patient using interpersonal dynamics to facilitate and effect therapeutic changes. Participates appropriately and actively in family conferences and/or family/patient training opportunities. Participates in case management needs as per requirements in individual departments Maintains established departmental productivity standards. Actively participates in departmental initiatives such as Performance Improvement Initiatives, and CARF, JCAHO, State Board of Health readiness. Provides direction to Certified Occupational Therapy Assistants by delegating appropriate patients, reviewing plan of care and providing clinical support as necessary. Provides day-to-day direction to rehabilitation technicians and volunteers. Participates in student programs, as appropriate, and based on level of experience and expertise. Attends all mandatory inservices and meetings and other inservices that are related to area of responsibility Provides in-service for staff after attending courses outside the organization. Adheres to and maintains all necessary competency requirements for job responsibilities. Performs other duties consistent with purpose of job as directed.

What We Are Looking For:

Education: Requires professional education culminating in a minimum of a Bachelor's degree in Occupational Therapy from an accredited academic program.

Licensure: Must be certified in the State of Indiana as an Occupational Therapist.

Experience: Prefer previous experience in an Occupational Therapy Clinical Setting but will accept a new graduate from an accredited college.

Other Job Requirements:

Excellent communication skills, both verbal and written are required.

Must be able to adjust of varying treatment loads and schedules.

Requires flexible work schedule including weekends.

Considerable initiative, judgment and problem solving required.

Assigned hours within your shift, starting time, or days of work are subject to change based on departmental and/or organizational needs.
