Job Description:
MAIN PURPOSE: Responsible for direct and indirect patient care
in meeting holistic needs of the patient and in directing,
evaluation, and referring of Agency nursing personnel.ESSENTIAL
FUNCTIONS:Management of Assigned Areas: Ensures compliance with
nursing care standards established by commissions, and federal and
state regulatory agencies, and adherence to Agency objectives and
policies pertaining to nursing administration. Manages resources,
facilitates concepts and approaches, reviews projects progress and
results; makes management decisions. Participates in the selection,
hiring and termination process of staff. Determines workload and
delegates assignments, training, monitoring and evaluating
performance, and initiating corrective or disciplinary actions in
accordance with Agency policy and in a timely manner. Responsible
for meeting the requirements for contracted staff. Responsible for
the scheduling and coordination of patient care staff. Acts as a
resource to provide information or determine the most effective way
of meeting the needs of staff, patients, or customers. Reviews and
assures accuracy of invoices related to areas of responsibilities,
in a timely manner. Responsible for supplies and PAR levels.
Participates in nursing leadership on call rotation schedule
related to clinical issues.
Clinical Services and Quality Improvement:Monitors clinical
documentation, assuring accuracy, completeness, and compliance with
licensing regulations, certification standards, and Agencys
policies and procedures, including but not limited to the
Comprehensive Assessment, Patient Medications, and Patient/Family
Plan of Care. Participates in the process for assigned areas during
any federal and /or state survey process or compliance activity as
needed. Acts as a contact for information relating to clinically
assigned areas for both staff and external sources. Ensures
appropriate scheduling for areas of responsibility and monitors and
manages appropriate staffing levels in accordance with Agency
productivity staffing standard. Participates in the pre-assessment
and admissions process. Participates in and is responsible for
quality assurance, improvement and utilization review activities.
Assists in coordination of IDG supervisory meetings. Responsible
for meeting the requirements of Hos/MCare/MCaid certifications and
recertifications. Monitors and ensures appropriate utilization of
patient related DME, supplies and biomedicals. Develops and
presents education programs for CHC and outside agencies. Completes
documentation related to any direct/indirect patient care contact.
Functions in the role of Case Manager as needed based on the
current census or staffing vacancy. Participates in patient care by
functioning in the role of Triage, home visits, and emergency
visits. Coordinates and prepares for the IDT meetings. Accountable
for Agency relationships in the LaPorte area including hospitals,
ECFs, group homes, and assisted living facilities. Attends ECF care
plan conferences. Policies and Regulations: Accountable for state
and federal regulations relating to hospice and home health.
Ensures appropriate utilization of levels of care for each home
health patient. Assists with maintaining quality assurance with
implementation of protocols, procedures, etc. Participates in the
development of policies and procedures as clinical practice
changes. Ensures and complies with HIPAA regulations. ADDITIONAL
FUNCTIONS: Committed to Agency resolution against fraud and abuse.
Knowledgeable in hospice philosophy, LCDs, and state and federal
rules and regulations. Arrives to work and meetings on time and
ready to work. Respects confidentiality of patients/families and
other Agency employees. Attends monthly nurses meeting and
educational meetings/conferences for professional growth as
required. Participates in nursing skills validations. Participates
in Agency infection control plan. Sees change as an opportunity and
maintains a level of flexibility, which allows for adaptation to
new ways of performing. Follows directions and policies that allow
for an organizational commitment to working under time pressures
without sacrificing quality. Performs other duties that may be
assigned.REQUIREMENTS: Possesses the ability to perform the
following tasks for varying periods of time: standing, walking,
sitting, lifting objects up to 75 lbs., pushing/pulling objects up
to 200 lbs., climbing stairs, stooping, kneeling, twisting,
bending, reaching overhead and in front of body, simultaneous use
of both hands, wrists, and fingers. Possesses the ability to
perform the following tasks for varying periods of time: speak in
conversational tones utilizing the English/American Language, hear
conversations as well as bodily function sounds with various
medical or non-medical devices, observe patients via touch
sensitivity that is strong enough to palpate various areas on the
human body and allow an accurate reporting of a pulse, see with
vision acuity: near, 20 inches or less, and far, 20 inches or more,
with or without corrective devices such as glasses or contacts,
have a depth perception of three dimensions, and a sharpness of
vision/focus with adequate peripheral vision to allow an acute
awareness of surroundings. Possesses the ability to work in a
variety of environmental conditions that include, but are not
limited to the following: adverse weather conditions, dust, fumes,
tobacco smoke, noise, closed-in spaces or rooms, and locations with
poor or improper lighting and glare. May be required to work in
shifts that may be steady or rotating. Possesses the ability to
operate and drive an automobile and maintains a valid drivers
license and current automobile insurance. Requires use of personal
vehicle.QUALIFICATIONS: Maintains current R.N. licensure required
in Indiana. Previous supervisory experience required. Previous
nursing experience in home health/hospice preferred
