Job Description:

Minimum: $35.42 -Maximum:$51.07 JOB SUMMARY Evaluates and treats patients referred to the Physical Therapy department. Documents patient evaluation, plan of care, progress and discharge status. Mentors Physical Therapists and PTA's. Provides leadership within the department with advanced clinical skills in such areas as wound care, student education, manual therapy, etc. as determined by the director of the department. Recommended certification if available in area of expertise. Ability and willingness to work in a variety of settings throughout the organization. JOB DUTIES 1.--Actively demonstrates the organization's mission and core values, and conducts oneself at all times in a manner consistent with these values. 2.--Knows and adheres to all laws and regulations pertaining to patient health, safety, medical information. Complies with regulatory agency (Medicare, JCAHO, CARF etc.) standards and regulations. 3.--Conduct evaluations consistent with and appropriate for patient diagnosis. Designs a treatment plan that is goal-oriented, based upon interpretation of evaluation results and patient's personal goals. Establishes goals that are functional, measurable and attainable. Performs re-evaluations per department criteria with modifications/updating of goals as dictated by patient's progress. 4.--Assists patient towards goal achievement through effective treatment plan and interventions. Administers individual treatment procedures. Develops home exercises and self treatment plans for the patient. Provides education to patient and family, including diagnosis and prognosis of therapy. Recognizes adverse responses or contradictions to therapy and adjust treatment accordingly. 5.--Makes timely and appropriate discharge plans and recommendations for the anticipated needs of the patient. 6.--Completes, in a thorough and accurate manner, evaluation reports, progress notes/flow sheets, discharge summary and additional required documentation (e.g. consent, staffing reports, rounds, attendance agreements, patient and family education needs assessment form).-- 7.--Includes documentation of the occurrence of education. Records occurrence(s) of education and/or home exercise program provided and reports patient/family return demonstration and/or comprehension of training provided.-- 8.--Adheres to documentation requirements by regulatory agencies (ex. records duration of specific treatment procedures in minutes). Submits daily charges that accurately reflect documentation within specified time frame. Assures that documentation is complete prior to discharging patient. 9.--Participates as requested in orientation of other staff, patient staffings and/or team meetings. Makes appropriate recommendations and referrals for additional services. Consults with other professionals and/or appropriately seeks assistance from co-workers as needed. Is flexible with hours of availability to meet department needs. Engages patient using interpersonal dynamics to facilitate and effect therapeutic changes. Participates appropriately and actively in family conferences and/or family/patient training opportunities and in case management needs as per requirements in individual departments 10.--Maintains established departmental productivity standards. 11.--Actively participates in departmental initiatives such as Performance Improvement Initiatives, and CARF, JCAHO, State Board of Health readiness. 12.--Provides direction to Physical Therapy Assistants by delegating appropriate patients, reviewing plan of care and providing clinical support as necessary. Provides day-to-day direction to rehab techs and volunteers. 13.--Attends at least 80% of all staff meetings. Attends all mandatory in-services and meetings and other in-services that are related to area of responsibility. 14.--Provide in-service for staff after attending courses outside the organization. Also provides at least one in-service yearly in area of expertise. 15.--Adheres to and maintains all necessary competencies required for job responsibilities. Assists in the development of competencies at least in area of expertise. 16.--Complies with all hospital policies and procedures. 17.--Provides mentorship for Physical Therapists by providing orientation and mentorship to new colleagues. Willingly accepts student interns. Participates in student program as appropriate and based on level of experience and expertise. 18.--Serves as clinical resource in area of expertise both internally and to outside agencies as requested. 19.--Participates in the organization and implementation of organization sponsored events such as Job Fairs, community educational opportunities, etc. 20.--Performs other duties consistent with purpose of job as directed. JOB SPECIFICATIONS AND CORE COMPETENCIES Education: Bachelor's degree in Physical Therapy from an accredited academic program required. Master's preferred.-- Licensure: Must be licensed in the State of Indiana as a Physical Therapist Experience: Minimum 5 years of experience with a well established clinical background in a variety of diagnoses and the related/resulting disorders. Clinical certification in area of expertise a benefit. Other Job Requirements:. Excellent communication skills, both verbal and written are required. Must be able to adjust of varying treatment loads and schedules. Requires flexible work schedule including weekends. Considerable initiative, judgment and problem solving required. Model identified behaviors for other staff to follow.
